Is Java working well on the system? See
Tools > Options > LibreOffice > Advanced > Java Options

You don't need a button there. There must be a navigationbar at the
bottom of the form-window. There you could save data by pressing the
save-button or by moving to another row.

Is the form connected to a table of the database? Are the textboxes
connected to fields of the database?
