Another idea I could investigate is to simply delete all
non-emailconfirmed users, which might be doable with a simple database

I am just suggesting that if we do go to manual account creation, requests for wiki access are for contribution only. So, it would be easy to identify spam request through a simple email question to the person involved asking them why they would need access. From what I see on the wiki, there is not that much contribution going on, so, there must not be that many legitimate account creation requests. So, it should be easy to identify legitimate requests.

Are there enough moderators for the NL groups if we were to go to manual account signups?

Just thought I would also pass this on, on one of my sites, the best captcha I had was an account creation question that just asked for people to type earlymusicnews backwards and I would change the word every 3-4 months. My spam was reduced to practically 0 overnight. I guess it just wasn't worthwhile for spammers to work it out by hand. I think I once read that there was a certain amount of seconds a spammer would attempt to create and account and that after this it was no longer profitable to work at it. Anyway, it seemed to work for me.
