you can use format codes. Format codes do not change the value of the
cell, but only how this value is shown. So the content of the cell would
be still the number 1 or 2, but you would read "malnutrition" and "normal".

Open Style&Formatting dialog, right-click cell style 'default', select

When you now enter a 1 into a cell, the input line shows a 1 and you can
calculate with the content 1, but in the cell itself you will see
'malnutrition'.

I have included the part [RED]Standard, so that all other numbers will
be in red to show, that this entry is not correct.
