you can use format codes. Format codes do not change the value of the 
cell, but only how this value is shown. So the content of the cell would 
be still the number 1 or 2, but you would read "malnutrition" and "normal".

Open Style&Formatting dialog, right-click cell style 'default', select 
'New'. Enter a name for the new style in tab 'Organizer',e.g. 
'myLabels'. In tab 'Numbers' select 'User-defined'. Then write the 
following into the input field 'Format'

[=1]"malnutrition";[=2]"normal";[RED]Standard

You can click on the "edit comment" icon to define an identifier for 
this format.
Then click the green check mark to add it. OK.
Now mark the area where you will enter the data and assign the newly 
created cell style.

When you now enter a 1 into a cell, the input line shows a 1 and you can 
calculate with the content 1, but in the cell itself you will see 
'malnutrition'.

I have included the part [RED]Standard, so that all other numbers will 
be in red to show, that this entry is not correct.
