Quite possibly, the configuration files from 3.5.x which point to the existence of macros in a structure (represented as a tree view in the UI under the macro management dialog) were not carried over when you upgraded to 4.0.x. I seem to recall that this is actually a known limitation (bug) of the migration tool used internally by LO to copy over the user configuration from the 3.5.x series to 4.0.x.

You should look for files bearing a xla/xlb extension in your previous 3.5.x installation and compare them with the ones in your current 4.0 installation.

Alternatively, deleting your current 4.0 user profile and restarting LO to generate a new one, might do the trick as well.
