goto chapter 15 - "using Forms in writer" and it explains how to do it
you also have to turn on a tool bar for it (who knew)

I would be so pleased to know if you get this to work.
These forms don't seem to be very useful in this case: I have an already
finished document and only need a few text-items replaced. The forms are
made from graphical elements which may or may not align with the text in
the document.
The documentation[1] says:
,----
| Forms are used in three ways:
| · To create a simple document for the recipient to complete, such as
| a questionnaire sent out to a group of people who fill it in and
| return it.
| · To link into a database or data source and allow the user to enter
| information. Someone taking orders might enter the information
| into a database using a form.
| · To view information held in a database or data source. A librarian
| might call up information about books.
`----
Perhaps I could use a form to put information into a database and use
fields in the writer document that refer to the database. That would be
very much like using the mailmerge tool.
Since there's some bug with remembering fields of data sources in LO,
it's probably not going to work any better than using the mailmerge
tool. Besides, I could enter the data directly into a database without
making a form.
So I don't know ... am I missing something?
