There are these principles for to determine header and footer:
(A)
You can use the predefined page styles `First Page`, `Right Page` and
`Left Page` and/or add own page styles for `Copyright`, `Dedication` and
`ToC` or whatever you need. In each of the page styles you can
individually decide about having a header and about having a footer and
about the content of header and footer.
(B)
You use one page style. In it you uncheck the options `Same content on
left and right pages` and `Same content on first page`. You can then
have different content in header and footer for first page, right page
and left page. This is usually combined with page layout `mirrored`.
There are these principles for to determine the change to a different
page style.
(C)
In tab `General` of a page style you can set the style for the next page
in field `Next Style:`. That mechanism is used in the predefined page
styles `Right Page` and `Left Page`.
(D)
You set the to be used page style in the properties of the first
paragraph of the page; in case a table is the first thing on the page,
then in the properties of the table. You will find the setting in tab
`Text flow`. In section `Break` check `Insert` with Type Page and
Position Before and check `with page style`.
(E)
You make the settings described in (D) not at the individual paragraph,
but make it in that paragraph style, that you want to use as first on
the page, e.g. paragraph style `Heading 1`.
You can mix the methods (A) to (E).
As you can see, there are numerous ways and possible combinations. There
is no general best method, it depends very much on your individual
requirements.
