When you wrote the book, did you define a bookmark as a target for each internal link?  I find that 
those internal links export to pdf just fine.

Internal links within a document will remain the same for anybody who received the pdf. As will 
external links to websites but NOT links to other files within your filing system as, for example, 
links to images which you might have in a different folder. Before exporting a document which 
contains links to images, you need to embed those images within the document.
