Steps are pretty simple.
Open Tools -> AutoText dialog (shortcut <Ctrl>+<F3>)
Click the 'Categories...' button to launch the Edit categories dialog.
Use drop list to select the user profile ../settings/user/autotext
Enter a name for the New category
OK out of the Edit categories dialog
Now add the AutoText (it must be entered from highlight in the document 
body of a Writer session)
Make a highlight selection on document page (if no selection made in 
document, only an Import is offered)
Open the AutoText dialog back up
Note the new (but still empty) category is listed
Select the new category if it is not active
Use the 'AutoText' button menu to add/
Once entries are in a category, the category will show a collapse arrow.
Suspect what is being missed is that when *creating* a new category you 
*must* specify a path where you have write access.
The default MyAutoText path is created with a new user profile.
