Hi Serena,
I never saw a template, primarily because a spreadsheet for
that is so easy to create. It depends how basic, or how elaborate you
want to be. A basic spreadsheet would consist of 3 columns: Date,
Systolic and Diastolic. Mine includes more columns: Time, Pulse, Oxygen
(for blood oxygen saturation), Weight, Glucose. Next, I plan on
including averages of each column.
