Hi Eric,
it's very, very unlikely that a document template simply disappears.
When you write that the document template is no longer present in the
list, I assume that you call your template via File>New>Templates.
Please check in the "Templates" dialog if the filter for "Text
Documents" and "All Categories" is set.
