Hi Ken,
I have to admit the various iterations of LO templates has consistently
stumped me however...
1. open the document you want to use as a template (remove all
superfluous text first)
2. Template->Save as a template
3. You will be presented with the boxes you referred to earlier
4. Highlight (eg.) "My Templates", click save and give the template a name
You then have the option of setting this template as default or opening
it manually to create a new doc.

I am using LO 5.1.2.2.U+ with Debian Jessie.
I cannot assign a template to a document. Whenever I try I get the
following message: "Property or method not found: DocumentInfo"
Also the "Manage Templates" function does not work for me. Whenever I
open it a new window appears, In it there are several small
rectangles, one of which is labelled "my templates". In that
rectangle are four smaller unlabelled rectangles. Whatever I do I
cannot find any way to assign any of my eight document templates to a
document.
Can anybody tell me what the problem is and how to fix it?
