That first screen is introductory. The question gets answered in detail by
responses to the questions asked on the second screen.

BTW, To the survey's author: If the answer to question 1 is Never then
question 2 potentially has no valid response.

       Maybe I wasn't allowing the correct (java or whatever) access;
          so here's a suggestion:
         how about a survey of simple text rather than whatever system is
now in play?
