A form is a ODF document. There are probably ways to hide the toolbars
by messing with the XCU configuration files of ODB file used to contain
your forms, queries, etc, or even with the general XCU files of
LibreOffice, but then you might mess up much more than just your form.
You may also be able to set the size of your form (dimensions x,y) via
the Form Properties.
Seems to me that a dialog box might be a better alternative for what you
want, but that would require you to mess around with the LO Basic IDE
editor.
