I have used frame for a such format for the binder few years ago. It
worked well for my purpose.
Draw one tall frame (same height as 'page text area') for subheadings.
Adjust page margin accordingly.
In my case, I used two different frame styles and two different page
styles for even/odd pages.
Left page frame style was anchored to 'left/top' and 'page text area'
and linked with 'Left Page style'.
Right page frame style was anchored to 'right/top' and 'page text area'
and linked with 'Right Page style'.
Insert a frame with correct frame style in every pages.
In this way, if you insert a page in the middle, your frame will shift
automatically depending on page #.
