One remark, the dialog where you edit the Index/Table where you have the
E# and E buttons. Those buttons are too small so you can not see in fact

Correct. That's a problem for a long time already.
Maybe an easy hack ;-)
This is using LibreOffice 3.4.4 on a RHEL6 using Gnome.
All versions as far as I remember.
