This sounds like one of the known bugs with the Ubuntu packaged version
of LibreOffice - the form navigation toolbar is missing. Judging by the
number of times it comes up, here, and on other lists, I guess the
Ubuntu packagers still haven't got around to fixing it.
The problem does not occur in the LibreOffice debs provided for download
via the LibreOffice website. If you want to install these debs, then you
need to remove the Ubuntu provided packages from your system first.
There is/was a page on the LibreOffice website which explained how to
install the debs and get desktop integration. It has even been explained
here on this list in other discussions, if you search in the archives of
the list. Otherwise you have to start LibO from the command line because
the debs install into /opt.
