I'm on Win XP with LO 3.3.0.
All those settings are set and seem to correct. I've made no changes there.
Just now I tried again and found spell check to be working but the insertion
point was invisible.
I got it back with a close/open.
Tried spell check again, and some other things, I guess, as the insertion
point went invisible but I
got it back with a save.
Can't reproduce this at will, though, so there must be more involved than
running spell check.

Have you gone into "Tools/Options/Language Settings/Writing Aids"
and checked all the
"Available language modules", "User-defined dictionary", and "Options"
that you want to use for Writer? Is the Toolbar listing of
"AutoSpellcheck"
and "Spelling and Grammar (F7)" shown properly and not "grayed out"?
What Platform are you using? Was there any language pack for you to
download and install? Some of the platforms have en-US, en-GB, and en-ZA,
available for the languages, while others do not show en-US in the list
at all.
I an running 64-bit Ubuntu/Debian installed with en-US language as my only
language, at this time, though the extension manager shows English,
French,
and Spanish dictionaries installed.
I have most of the "Writing Aids" checked to be used, and I have no
problem
seeing red markings for words not known in the spelling dictionary and
blue
markings for grammar problems.
Hopefully you have not checked some optional part of the "Writing Aids" or
some other simple thing was not done or turned on that is causing your
troubles.
