My question : is the problem in 11) and 14) ?

If one is starting a new document, _Hierarchical_ is an acceptable
default. (The exception is a new document that is created from a
template that includes text.  In that instnace, _Applied_ makes a better
initial default.)

However, if one is editing an existing document, _Applied_ makes a much
better default, because it shows the styles that have been used in the
document.
