If you do not compile LibreOffice yourself, but only want to add an icon
via the customize dialog, then the name does not matter.

In case you want to compile LibreOffice yourself, you find all needed
information in the README of the 'icon-themes' folder.

Short answer: All command characters to lower characters and prefix with
lc_ for large icons and sc_ for small icons. In your example you get
lc_pasteunformatted.png and sc_pasteunformatted.png
