thank you for the detailed report!

I can reproduce that PAC reports such an error when setting a background 
image in Impress.

When doing basically the same in Writer, I see no such error, however.

I'm not an expert when it comes to PDF export, but this sounds like the 
Impress PDF export might indeed not tag something as it should.

Could you please create a bug report in Bugzilla [1] to keep track of 
this? (You can simply copy your description from the email, which 
already nicely describes the problem and steps to reproduce.)

If you add "139007" to the "Blocks" field of the bug report, it will 
automatically be added to the PDF accessibility meta bug tdf#139007 [2], 
so people subscribed to that one will also be notified.

Since you mentioned https://check.axes4.com : Is there a way to see a 
detailed description for the issues found on that website, too, similar 
to PAC?
(I didn't see a way to do that. I only see the number of failures listed 
for each category - like 501 errors related to Content - but then had to 
switch to PAC to see what the actual problems are and am wondering 
whether maybe there is a way that I was just missing.)
