thanks a lot for the writeup and for handling the issues! I know you 
stayed up late at night to run the upgrade outside European business 
hours and then were woken up to jump in after very little sleep...

It seems indeed we've hit an unfortunate issue that was very hard to 
identify upfront - what we could look into for the future is running one 
of the web server benchmark tools before going into production, to add 
one more layer of double-checks, but of course that doesn't give a 
guarantee either.

Thanks again for the summary and sharing the details, and for working on 
the issue!
