Header Versioning: Numbers at all levels, and all levels displayed.

Header Style: No indentation at any level.

Suppose I have the following:

1.0 A
1.1 C
1.1.1 D
2.0 E

What I would like to do is insert a header between A & C to get:

1.0 A
1.1 B
1.2 C
1.1.1 D
2.0 E

What I get is:

1.0 A
1.1 B
1.1 C
1.1.1 D
2.0 E

If I insert an entry at Header Level 1 instead of Header Level 2 I get:

1.0 A
2.0 B
1.1 C
1.1.1 D
3.0 E

I've tried Tools->Update->All, and I have looked at Tools->Header Numbering to see if there was any 'trick' there. Doesn't seem to work. So, what have I done wrong and how do I fix it?
