So, what I have done is to remove the QuestyCaptcha and replaced it with
ReCaptcha again, to avoid problems with NLC projects. Let's see if this
brings in more spam again - QuestyCaptcha so far helped rather well.

I also added some more extensions, we now have the same set of antispam
extensions as Wikipedia uses. I guess it's now on to sime more
fine-tuning. I read the MediaWiki antispam documentation about 9 months
ago, and had back then most of the things in it that they recommended.
I'll read through it again, chances are that since then some more best
practices came up that help in combatting spam.

As last resort, we can always at least temporarily move to manual
account approval. This is tedious, but can come to rescue in case the
spam issues grow faster than we can act.
Well, I've stopped my contributions here, but if you need me again,
don't hesitate to ping me directly, I can work on the wiki early in the
morning.
I will keep you posted, and investigate further.
