I updated the form. We have a drop down field with two values here now.
The default value is "No".

I added also a drop down field for the ways of payment. The values are
only an example list. I'll update the list, once I get the feedback,
which ways of payment are available.

I added a field with that explanation on top of the fields for bank data.
