Just tried applying icons. I used the stock "Default", "Italics", and "Bold"
Icons offered by LO. However, they're not showing up in the toolbar, only
text. The Icons option is chosen. Also tried "Icons & Text" option.
They have no default icon. You need to assign icons manually. Do not do this
under icon size "automatic", but set icon size fixed and then chose the icon
which has this size for the new command. You need to do it for each icon size.
