My approach is to right-click on the new form in the main database
window and choose 'Edit'. 

When the form opens select 'Form' from the main menu and ensure that
'Design Mode' is select and select additionally 'Form Properties'. 

A 'Form Properties' window should open which allows you to edit the
properties of the form. Under the 'Data' tab you can enter the table
name (or query, or an SQL command) which contains the data source for
the form.

I hope this helps and corresponds to how your LibreOffice is
configured.
