A template really isn't needed.  All you need are:
- A title in one of the cells on the first row.  For example "My Blood Pressure Readings".
- Leave a row space for readability.
- On row three, add these column headings starting with column 1:
  "Date", "Time", "Systolic", "Diastolic", "Pulse", "Notes"
- You may want to make the column headings bold for readability.
- Start recording the readings in the spreadsheet.

Once you have a few readings, you can highlight the range and create a chart that will show you how 
the blood pressure is varying by day or time, and any other special situation noted in Notes column.

Very easy actually.
