maybe you've uncovered a bug in your version ;-)
If the document is as it was when you saved it, then check to see
which extension you've saved it as ...
maybe 'search'ing would pull up the /each of the document(s);
then you'll be able to see if there are more than 1 & where each
is located.

Hi,
I have a document in Writer. I saved it with a name.
When I open it however, the open document shows
"Untitled" at the top instead of the name it was saved
with. Then, as I add to the document & save, I am asked
what I would like to save it as.
So again, I save a document as mydoc123.odt. Later I open mydoc123.odt.
Once opened, instead of mydoc123.odt showing at the top as the title,
it says "untitled". When I update the document and save, I have to
name it again. Why?
