I dunno a good answer to this. Maybe someone else on the list will come up
with something better. In the meantime maybe one of the chapters in
documentation might help;

Maybe the Writer Guide chapter 11, "Mail Merge". The more obvious choices
are chapters 14 and 15 about Fields and Forms.
