Hi,
check the extension of the file.
I suspect that you are opening a document *template* which I noticed has
such a behaviour. After you save the file as non-template file it should
work.

My problem is that unless I open a file using LibreOffice's own file
dialog screen, the document does not open properly, meaning the file
name appears as Untitled!
Double-clicking on a file icon or name in Windows explorer opens the
file, but the file is named Untitled in the bar at the top.
Subsequently, when I want to save it, I need to navigate to the
sub-directory where the file was previously saved, each and everytime.
This is a real pain; it never used to happen in Open Office.
I am using Win XP and work mostly with Word and Excel files.
Any clarifications/work-around appreciated.
