The forms in the embedded database you attached to issue 116878 work for me in OOo 3.3 and LibreOffice as well.

Tested under XUbuntu(xfce) with Oracle Java 1.6.0.22

You are right, stating that the form documents are Writer doucuments and LibO has development capacities for Writer, but forms with controls can be attached to any type of office document. They are more like OLE objects, charts or math formulas. The whole functionality of a bound form is related to the functionality of the underlying database and its driver.
