In a nutshell, please send
- your receipt for travel and hotel
- the filled in SPI form from
https://wiki.documentfoundation.org/images/d/d9/SPI_reimbursement_request_editable.pdf
via e-mail to treasurer@documentfoundation.org. To get the required bank data, it's usually sufficient to ask your bank what to provide when receiving money from the US.
I'll arrange travel refunds with SPI then.
