p.125. Reference to p.125! Should say "The link option is discussed below"

p.132. Green resizing handles. I thought we had agreed to call these "colored" as they appear cyan in some systems.

p.135 Group 1). ditto.

p.137. Reference to Fig 156 should be Fig 130.
  "They are is not retained..."
p.139. Reference to Fig 160 should be Fig 132.

p.140. Reference to Fig 157 should be Fig 133.
  Aligning graphics 1) Green handles - see above.
  Reference to Fig 259 should be Fig 134.

Could someone explain to me why we have so many bad references? I thought that LO updates figure numbers whenever new figures are inserted and then also updates the cross-references to these numbers.

It's a bug that occurs when some of our chapter files are concatenated
into a book, whether they are joined using a master document or by
inserting files. The bug is difficult to reproduce and goes back to
earlier versions of OOo (i.e., before LO). Various people have done
some forensic work to try to determine what is going on to cause the
bug, so the programmers might have a chance of fixing it.
