(2) Can anybody get the key combination Ctrl+Alt+E to work on Windows
10? It works fine on Linux where is is equivalent to Tools> Extension
Manager and displays the Extension Manager dialog.

Works here with Version: 6.4.1.1 (x64)
Build ID: 56f3c78975db08733f771c53643b5d1aa7c57567
CPU threads: 12; OS: Windows 10.0 Build 18363; UI render: GL; VCL: win;
Locale: ru-RU (ru_RU); UI-Language: en-US
Calc: CL

(3) The key combination Ctrl+U appears to be a shortcut to Format > Text
Single Underline but is not shown on the relevant menu option. Should
it be shown there? (Note – if you hover over the underline button in the
Formatting toolbar, then the Ctrl+U shortcut is displayed in the tooltip).

The menu item is "Single Underline" (.uno:UnderlineSingle). The toolbar
element is "Underline" (.uno:Underline). They are different commands
with different keyboard bindings.
