I updated it according to what we discussed on IRC:

Merged the various ways to insert media, moved 'Gallery' from 'Tools'.
The entry to add media is at the top.

Keep the OLE objects name. Rename 'File...' to 'Text File...'

Dropped Plug-ins, since it doesn't work.
